1 hour time slot to assist you in making a referral to ART
Service Description
Applied Abilities is providing a free service to the community, schools and families, by assisting families in making an online referral through to ART (Assessment and Referral Team). What is ART: ART is providing one-on-one application assistance to eligible Queenslanders. ART is a free individualized support service and can help you fill out your application form, talk to your doctors and specialists, submit your NDIS access application and help you track its progress. (https://queenslandcommunities.engagementhub.com.au/art) Who can access Art: The Assessment and Referral Team (ART) is taking referrals to support at-risk children and young people aged 7-25 years who: - Identify as Aboriginal and/or Torres Strait Islander. - Identify as culturally and linguistically diverse. - Identify as LGBTQI+. - Are engaged with, at risk of engaging with, or transitioning from the Justice or Child Protection systems. - Are disengaging from education or transitioning from school. - Experience, or are at-risk of experiencing domestic and family violence, homelessness, social isolation and/or harm. - Have caring responsibilities. - Are living in regional and remote areas (https://queenslandcommunities.engagementhub.com.au/art/news/who-can-art-help-access-the-ndis) Applied Abilities, is assisting the community, by helping families who need support in submitting the online referral form to ART. We are doing this, as we have seen a gap within our community, where youths who have a disability and are about to disengage from school, families know about the NDIS, but dont know how to access it. We want to assist and this is an obligation free service.
