Referral through to ART

1 hour time slot to assist you in making a referral to ART

Service Description

Applied Abilities is providing a free service to the community, schools and families, by assisting families in making an online referral through to ART (Assessment and Referral Team). What is ART: ART is providing one-on-one application assistance to eligible Queenslanders. ART is a free individualized support service and can help you fill out your application form, talk to your doctors and specialists, submit your NDIS access application and help you track its progress. ( Who can access Art: The Assessment and Referral Team (ART) is taking referrals to support at-risk children and young people aged 7-25 years who: - Identify as Aboriginal and/or Torres Strait Islander. - Identify as culturally and linguistically diverse. - Identify as LGBTQI+. - Are engaged with, at risk of engaging with, or transitioning from the Justice or Child Protection systems. - Are disengaging from education or transitioning from school. - Experience, or are at-risk of experiencing domestic and family violence, homelessness, social isolation and/or harm. - Have caring responsibilities. - Are living in regional and remote areas ( Applied Abilities, is assisting the community, by helping families who need support in submitting the online referral form to ART. We are doing this, as we have seen a gap within our community, where youths who have a disability and are about to disengage from school, families know about the NDIS, but dont know how to access it.
